Modules Description
Medical Appointments
Module
This module is designed
to help the MD avoid the five – patients – today – fifty –
patients – tomorrow situation. It
provides easy to use tools to help distribute appointments evenly,
according to available time and resources.
The process, designed both for the Reception Desk
and for the MD desk, works like a flip – page calendar: by entering the
date you get the whole day of appointments in your screen showing time of
the appointment, name of patient, record number, patient telephone and
service to be provided. With
the touch of a key you flip the page and get next day details on your
screen. Addition of
appointments is an easy question and answer process controlled by the
system. Or, the MD can select
next appointment date with a single keystroke!
Maintenance of the appointments calendar is as easy
as entering the new date for a change of dates. The system will allocate the first available slot according
to user – defined parameters, e.g, five services/appointments per hour.
Or, the operator can negotiate a more suitable hour for the patient
or for the office practice. Holidays and vacations are entered in advance to prevent the
system and the operator from using those dates.
The system will automatically print appointment
reminders in post cards 10 days (user defined parameter) prior to the
appointment date. It will
also print, daily and automatically, post cards for patients that missed
their appointments that day, and it also prints other types of follow-ups,
as requested.
Examination Room Module
This module is designed
to host the patient through each step in his/her visit to the office.
Reception Desk – records are updated with
patient’s data, medical plans, medical history, etc.
Patient signs a computer – printed form with all data, certifying
correct information was provided, for MD’s legal protection purposes.
Pre-Exam Room – data is caputred on exams and
tests: vision acuity,
refraction, tonometry, lensometer, etc.
If the equipment used for the exams provides computer output
capabilities (Nidek, Humphrey and others), they can be connected to the
computer, making the process fully automatic.

the MD will select diagnoses from his pre-defined tables, combine them as
necessary, select and update a progress note, tentatively assign next
appointment date and select applicable services or procedures. The
system will provide and validate appropriate ICD – 9/ CPT – 4 codes,
according to pre-defined tables for MD’s particular practice.
Exit Module
This event – driven
module usually takes place at the Reception Desk, where the system
enforces an ordered exit process.
Formalization of next appointment date – The system
will not continue the exit process until this step is completed.
No patient shall leave the office without a new appointment, at
least for next year follow – up.
Printing of visit documentation – This includes
an appointment card, the progress notes, prescriptions for glasses, for
medicines, patient instructions, MD referral documentation. All these documents can be printed and re-printed from
computer’s data, as needed or required.
Invoicing – Invoice details are first displayed on
the screen, complete with billing, plan – approved and deductible
amounts. The user must then
update amount actually paid by patient and any other amount, as
applicable. If payment does not fully cover the deductible, an invoice
will be printed for patient. If plan accepts electronic billing (Medicare
and SSS at the moment), the system advises that patient must sign
signature – on – file register. Forms
for other plans can also be printed and even a receipt for the patient can
be printed optionally.
Transactions are automatically generated and posted
to accounts receivable as applicable, for the patient’s primary plan,
complementary plan and also for the patient, if needed.
All transactions are printed and balanced at the end of the day on
the Invoice and Collections Register.
File Maintenance Module
This module provides the
capability to maintain all files in the system: patients database, medical
plans files, customers files, other reference and auxiliary files.
The module is also used by the MD to define his/her
practice standards: progress
notes, diagnostics, services and procedures, prescriptions and patient
instructions, etc.
Reports and Queries
This module provides
reports and inquiries on the patient database, billing and collection data
receivables due, etc. It also
provides for the re – print of prescriptions and progress notes, upon
request.
The Search and Analysis sub-module is of
particular importance, as it provides a powerful means for searches
through the database. This is an extremely important tool in medical practice
nowadays, as the MD is not only ethically bound to follow up patients
at risk, but
may also be legally liable for NOT doing
so.
For instance, the MD can
recall the lady from Orocovis with diabetes and very high intraocular
pressure. The MD knows she
was due for a follow up visit but has not seen her lately.
The MD can not recall any other information about the lady.
This situation is quite common in modern medical practice, where it
is impossible to remember all details about thousands of patients.
With CDI Medical Records System you can request a search like:
All females from
Orocovis, age over 60, with intraocular pressure over 20, and with a
medical history of diabetes.
Other search examples:
Intraocular lens brand
“X”with serial number starting “XYZ” has been recalled from the
factory. You certainly want
to know which patients, if any, were
implanted those lenses. Search:
patients with service procedure = “Cataract operation”, lens =
“X”, s/n begins
“XYZ”.
For statistical
analysis: how many patients with original complaint of
“Eye Pain”were eventually diagnosed
“Glaucoma”.
Practice analysis: how
many patients were referred to Dr. Rivera during last year. How many
times MD prescribed “Xyztin” during last six months.
